All athletes planning to participate in a Mira Loma athletic team must have an athletic clearance. This requires...

1) Filling out the online forms 

2) Upload a cleared & updated SJUSD physical

3) Notify Athletic Director if your student is a transfer student (9th-12th grades)

No athlete may attend tryouts, practice(s), or games without the athletic online forms and SJUSD physical completed.  

 

Please read through the handbook for the additional CIF in-season athletic requirements that include but are not limited to GPA, transfer eligibility, and sportsmanship.

Thank you for your interest in becoming a Mira Loma student-athlete. Please use the following link to fill out all required online paperwork SportsNet Inc.  

 

If you are having difficulty navigating through the program Here are some extra instructions on how to complete the online athletic clearance and the REQUIRED physical form. 

 

Please note that a completed San Juan Unified School District physical form must either be uploaded during the online clearance process or handed into the athletic director’s office.  Physical forms must be completed and signed by a doctor in order to be valid.

Don’t forget to select any sports your athlete is interested in.
