All athletes planning to participate on a Mira Loma athletic team must have an athletic clearance.

This requires...

1) Filling out the online forms 

​

2) Upload a cleared & updated SJUSD physical

​

3) Notify Athletic Director if your student is a transfer student

(9th-12th grades)

No athlete may attend tryouts, practice(s), or games without the above done.
